Multilobar Resections and Hemispherectomy

چکیده
Multilobar resection and hemispherectomy are surgical options available for the treatment of medically intractable seizures arising from a diffuse area of epileptogenicity that remains unilateral but extends beyond one lobe. The nature and extent of the underlying pathologic process, the patient’s neurologic status, and the results of preoperative neurophysiologic and neuroimaging studies together determine which surgical procedure should be recommended. For clarity, hemispherectomy and multilobar resections are presented separately in this chapter.

Which is the Most Appropriate Disconnection Surgery for Refractory Epilepsy in Childhood?
Children with unilobar or multilobar pathology issuing in refractory epilepsy are potential candidates for surgical treatment. Extensive surgery results in good seizure control, but it also increases the risk of neurological deficits as well as motor and mental problems. We reviewed the cases of 19 children with refractory epilepsy treated surgically at Osaka University Hospital. Four of the 19...
متن کاملAge and etiology as predictors of language outcome following hemispherectomy.
We report on the effects of etiology and age on the linguistic outcomes in a large pediatric hemispherectomy population. Four populations were considered separately: cortical dysplasia (multilobar involvement), Rasmussen's encephalitis, infarction as a primary etiology and, fourth, children who failed to develop language, regardless of etiology. We argue against the 'the-earlier-the-better' hyp...

متن کاملEpilepsy surgery in children and adolescents.
There has been a considerable increase in interest in the surgical management of intractable epilepsy in children. Better methods of diagnosis and improved knowledge of natural history and results of surgery have been major factors in this phenomenon. The results of surgery in 76 patients who were 19 years of age or younger are reported. Success rates, defined as no postoperative seizures with ...
